Preparing for surgery or a medical procedure can be a stressful experience – preparing for post-discharge care shouldn’t be. Centre Care can assist with your rehabilitation needs through our Pre-Plan Your Recovery Program.

Pre-Stay Registration

Pre-stay registration assists with a smooth transition from the hospital or surgical center to Centre Care. Whether you need outpatient therapy or a short stay in our rehabilitation suites, we can assist you with the pre-stay planning process. Choosing the service you need in advance, completing necessary paperwork and meeting your care team before your procedure provides you with the peace of mind that once your procedure is completed you can focus your efforts on your recovery.

Tour the Community

A tour of our state-of-the-art rehabilitation gym will show you where each phase of your service will take place. If a short stay at our facility is part of your post-discharge care plan, your tour will also include our rehabilitation neighborhood complete with private suites, separate dining bistro and private outdoor courtyard.

During your tour, our experienced business office staff can review your insurance coverage with you and answer any questions you may have.

Meet Your Care Team

Establishing a relationship with your care team in advance may further alleviate the anxiety that comes with planning for a surgery or medical procedure. Your individual needs and preferences can be discussed so that our team is ready to assist you in meeting your goals.

Home Evaluations

Our therapy team can provide an in-home therapy assessment prior to your return home. Home evaluations focus on the physical environment and the individual’s ability to navigate the home environment safely.

Discharge Planning

Your care team will assist with your smooth transition home. This includes assistance with scheduling home health services, scheduling follow-up physician appointments and ordering equipment needed for your transition home.

Outpatient Therapy

If your post-discharge care includes a short stay in our rehabilitation suites, you may choose to continue seeing your same therapists through our outpatient therapy program. This promotes continuity and allows you to continue working with familiar faces to achieve your therapy goals. Our outpatient therapy program is available to everyone in the community, whether they have had a short stay here or not!
